Long-term maintenance of immunity in patients with common variable immune deficiency by plasma transfusion.

چکیده
The AIDS crisis and the fear of blood product contamination stimulated the development of a designated plasma collection and transfusion for patients with common variable immune deficiency. Four patients have been maintained over 1 year; 5 years and 10 months; 8 years and 7 months; and 12 years. A new crisis affecting commercial IgG manufacturing, owing to recalls, has made the life saving product scarce, threatening patients' health maintenance. Our experience over a long time has demonstrated that plasma transfusion is equivalent to management with commercial IgG. The program of product collection and patient management includes testing the collected plasma for IgG content, selecting ABO compatible plasma, and following patients with IgG trough levels, prior to each transfusion, to assure sufficient immune globulin administration.

The Iranian Blood Transfusion, Donor Safety Program: Effect of Long-term Plasmapheresis on Plasma Proteins
Background: Plasmapheresis is a well-recognised method for harvestingplasma. It was introduced in the 1950s and is currently employed worldwide. This method has been in use in Iran since 1979. Objective: To evaluate the efficacy of donor safety program used at the Iranian Blood Transfusion Organization. Methods: A comparison is made of the protein concentrations between the first-time and lon...
متن کاملIVIG Anaphylaxis in a patient with Common variable immune deficiency disease (CVID).
Common variable immune deficiency disease (CVID) is one of the most common primary immunodeficiency disorders. It causes reduction of immunoglobulin levels and specific antibodies production and enhancement of recurrent and chronic infections risk, especially respiratory infections and gastrointestinal tracts. Here, we report a case of CVID in a 3 years old male with recurrent hospitalizations ...
متن کاملLong-Term Treatment and Transfusion of Normal Blood Components Following Tolerance Induction in Patients with Anti-IgA Anaphylactic Reactions.
BACKGROUND In general, patients with significant anti-Ig-A do not tolerate intravenous (i.v.) administration of normal blood products. Here, we present our experiences in the induction of immune tolerance (IIT) and long-term treatment in a series of such patients affected in such a way. The question whether blood components from IgA-deficient donors are required will be discussed. METHODS Ten...
متن کاملTACI deficiency impairs sustained Blimp-1 expression in B cells decreasing long-lived plasma cells in the bone marrow.
Deficiencies in transmembrane activator and CAML interactor (TACI) result in common variable immune deficiency, a syndrome marked by recurrent infections with encapsulated microorganisms, impaired production of antibodies, and lymphoproliferation. How TACI promotes antibody production and inhibits lymphoproliferation is not understood. To answer this question, we studied the generation of immun...
متن کاملLong Term Follow up Study on a Large Group of Patients with Congenital Factor XIII Deficiency Treated Prophylactically with Fibrogammin P®
Factor XIII deficiency (FXIIID) is an extremely rare hemorrhagic disorder with a prevalence of 1/3-5 million. Management of disease is performed by fresh frozen plasma (FFP), Cryoprecipitate (CP) or FXIII concentrate (Fibrogammin P®). Our objective was to assess safety and effectiveness of Fibrogammin P® in patients with FXIIID. For this purpose we designed this long-term follow up study on a l...
